Charming country bumpkin Duane Bradley takes a motel room in New york with no other luggage then a basket. In a flash back-series we learn it contains his surgically removed Siamese twin who is not only physically deformed so badly the doctors hesitated to consider him a human, but is also the vindictive drive of their trip, with the purpose to kill off all those he blames. But in the reception of one of those doctors, Duane gets his first ever date, with the receptionist, and wants to start a positive life too - when the freak twin escapes, these scene is set for a grim finale.

They wouldn't let us go to school or anything. They kept us hidden. We were the 'big family secret'. everybody hated us, except our Aunt. See, he likes the dark. He doesn't like to be seen, not even by me sometimes. And you know what else? Duane Bradley: [Casey shakes her head] He talks to me Duane Bradley: [Points to his head] Up here. Duane Bradley: Without words. I just here him whispering in my brain. Sometimes he talks for hours and wouldn't shut up. i used to be able to talk to him like that, but that was when we were still connected. Out Aunt said it was our special gift. But since we've been separated, I can't do it anymore, bu he can still do it to me. In fact, he's even better at it and he always knows what I'm thinking. Casey: Duane, you're giving me the creeps. Duane Bradley: They didn't want him to live. But he fooled them, he just got stronger. If you only knew what it was like. Duane and I kept hidden from everyone. We're both so messed up. I don't know which one of us is worse.

Movie Trivia:
- When Duane checks into the Hotel Broslin he takes out a wad of cash. According to Frank Henenlotter, the film's director, this was the film's entire budget.

To try to make the film appeal to a comedy crowd, the original distributor cut all the gore scenes out of the film. They were eventually put back in and re-released in theaters with the subtitle "The Full Uncut Version!"

Most of the credits that appear on the end of the film are fake. The crew was very small and rather then repeat the same names over and over again they decided to just make up names.

The Bar scenes were shot in a S&M club in Manhattan which is now know as "The Hellfire Club". The bar was so big that they also used it as the Bradley's basement and the crew had to hide all the sex toys including a large swing (which is still in the club to this day). The original buzz saw used to kill Duane and Belial's father used to hang above the door of the club until it was eventually stolen.

The Hotel Broslin doesn't actually exist, and the main lobby parts were filmed in a propped-open service elevator on Franklin Street, which is in the Tribeca section of New York. The scene at the end where Duane and Belial are hanging from the Hotel Broslin sign was actually filmed on an exterior building Downtown on Hubert Street, which is near Franklin Street. All the interior shots were done at several other locations, which were mainly friends' houses, etc.
