Returning home from a championship game, a group of varsity basketball players, cheerleaders and coaches become stranded on the infamous East 9 Highway - only it's the cunning Creeper who has actually crippled their bus. As its 23 horrifying days of flesheating come to an end, the Creeper has embarked on its final voracious feeding frenzy in Poho County. As night falls, the terrified group of young athletes must fight their own fears and prejudices and come together in a seemingly hopeless struggle against a winged nightmare, hellbent on stockpiling as many victims as it can on the ultimate night of its grisly ritual feast.

Director Victor Salva usually makes a cameo appearance in his movies, but because this film has no good spots for him to appear, his face can be seen very briefly on a magazine when the Creeper reaches through the roof of the bus.

Victor Salva wrote the whole "Every 23 years for 23 days it gets to eat" rule in the first movie so there would be no sequel unless the movie was set in the future, and knew the studio wouldn't want that. However Francis Ford Coppola found an easy loophole in the rule for the sequel: set it during the same 23 days as the first movie. So this film is set on the 23rd day for the purpose of not making another sequel.

In the first scene, before the Creeper appears, Jack Taggart Jr. is working underneath a truck. The device he uses to slide underneath it is commonly referred to as a "creeper".
